Part 2 SAFETY FORM is required if you plan on participating in tours. No need to fill it out if your car is in the restoration stage or for non- club riding or shows.

Part 2 SAFETY FORM- No need to fill this form out UNLESS you plan to tour with us on a club sponsored tour. In that case, complete the form and submit it at the tour- prior to the ride. We are required by our club's insurance company and the National Model T clubs to have these forms on file for added liability security.
